I teach middle school, and with all of the technology push they're doing these days, this is a MUST. My students have SO many different passwords for different websites, so I figured this year I'd have them paste this into their math journal. As we have different passwords come up, they will have a place to write them. At the same time, they will have a place to look for them when they forget!

Send this page home at the beginning of the school year. This will give you information that will help you throughout the year. After you get it back, copy the 'Parent Contact Log' on the back of the page. Keep them all in a binder. A super easy way to keep up with parent contact information, and log communication as well.
