As much as I hate to have to send home behavior reports, these comes in handy to just make a couple circles around or check marks next to the kind of behaviors being displayed and write a quick comment. This is a simple half sheet or two reports on one letter size page. One note for the parent and one for the school file. It is a simple form, very little writing required, lots of just circle the answers. I have been using this form for many many years. I love it's simplicity!
