My start of the year survey with 40 questions. This is a word document. Therefore everything can be edited. I use this with my students RIGHT when they get into my classroom. It gives them something that is low pressure to do and helps me get to know them right away. I've also updated the generic questions to fit with what kids like now (instead of TV show, it is Youtube channel, etc).
