The Problem Behavior Frequency Data Collect Form can be used to help identify the occurrence of problem behaviors in the classroom. This Word document can be edited to document specific behaviors and the student's schedule. The purpose is to help problem-solving teams and teachers gather baseline data about a student's behavior and then monitor progress. Frequency data can be collected by class or by AM/PM in each area of the defined problem behaviors.
Use this to help take data on a students' ability to generalize their use of calming strategies in the classroom. When a teacher notices that the student appears nervous/worried/upset, the teacher can prompt the student to complete the "I feel" section of the form. This is a great tool for students to rate their level of anxiety with teacher corroboration and support.
