This form can be used for your Parent Teacher Conferences. Print one form for each student. Keep the form handy so you jot down notes about each student. Then, when you are ready to prepare the forms for conferences, you can highlight the phrases that pertain to each student and add comments to reference during the conference . Make a copy of the completed forms so you can give one to the parents to keep.

This form can be used to learn about your students the first day or first week of school.

This template can be used if your student is absent from school. There are two versions: one with religion as a subject taught for Catholic schools and one without. Just print out the template and fill in any work completed during the school day along with any homework. You could have your "Homework Helper" fill in the sheet too to save yourself time.
