An ABC (Antecedent, Behavior, Consequence) data collection form was designed for use by the general or special education teacher. It provides a brief description of ABC components for teachers that are less experienced with ABC data collection. This is not all-encompassing but will allow for a quick start to behavioral data collection. Can print double-sided. I recommend only printing single-sided for easy copying or filing.
