I use this sheet at the beginning of the semester with my science students. It allows me to see what their timetable and after-school activities look like, as well as their expectations for taking the course. On the back of the student information sheet, I print a communication log template. All I have to do is collect the student information pages on the first day of class, throw them into a folder or binder, and I have a quick reference of 1) who to contact at home; 2) how to contact them;
