I created this chart to record the several kids I have with major behavior issues in my class. I needed something quick and easy that I could use with multiple kids. Just add the kids names, change the behaviors you are documenting, if needed, add a date and make a tally for every time you see a specific behavior. So much easier than having to write it all out every day.

Behavior note to send home

This is a quick and easy note to send home to parents about their children's behavior. I use this for my extreme behavior children daily and the other children as needed. There are positive as well as negative behaviors listed so if you have children who always get a negative note, you can add those positive behaviors seen that day. I left this in the .doc form so you can change the behaviors to those that you need yourself in you own rooms.
