I created this chart to record the several kids I have with major behavior issues in my class. I needed something quick and easy that I could use with multiple kids. Just add the kids names, change the behaviors you are documenting, if needed, add a date and make a tally for every time you see a specific behavior. So much easier than having to write it all out every day.

Quick and easy way to print out a monthly attendance form. I update monthly and print out for my small resource groups. Just type in the month, dates, subject areas and student names then print and you're good to go! I put a check in the box if the student is here, O if absent, T if tardy and c/a if out due to a class activity
