This template is meant to be used to type your notes for any co-teacher meetings, IEP/504 meetings, and then used to write any initial IEPs or 504s or updates to them. Document includes the sequential headlines within FRONTLINE- IEP DIRECT to help you organize notes and comments to copy and paste into the document. Additionally, it has data charts for various assessments (DIBELS, Core Phonics Screener, LETRS, Raz-Kids Running Record, and Reading/Math iReady Diagnostic).
