This is an EDITABLE guide to be used when doing initial/ongoing observations of a student who is Deaf/Hard of Hearing/may qualify for D/HH department. I use this when observing a student for the first time to help assist in D/HH placement. This form can be included in a student's CUM and information can be gathered and transferred to an IEP to support placement and services.
Use/ Edit this checklist to collect data regarding a student's mastery of the alphabet and numbers 1-20. Can be used to monitor receptive/expressive/both. I use this with Pre-K to 1st graders before creating an IEP goal. Can be used with signing or oral auditory.

Use/ Edit this form to collect pre/post data regarding a student's expressive/receptive language in the areas of days-months-colors. I use this when monitoring if a student can sign/verbalize, or receptively identify if I provide the sign/word prior to creating an IEP goal. Then I test them again at the end of the IEP period to show mastery. The form can be edited in order to create extra data tracking boxes if you would prefer to test them quarterly.
Use this to document parent communication such as date, who you spoke with, did they answer the phone, summary of what was discussed. Great way to document for IEP/CUM folder purposes!
