Digging Into First Grade

Have parents fill the sheet out at Meet the Teacher Night or Back to School Night to get all the information needed about students. Sheet includes: -Name and nickname -Birthday -Allergies/Medical conditions -Parent/Guardian name, home, cell, and work phone #, employer, e-mail address, and preferred communication method -Emergency contact information -Dismissal information (bus, walk, car, daycare) -A section for any other details the parent wants to include

Use this sheet to prepare for parent teacher conferences. Write in the student's strengths and areas for improvement. At the end of the conference sign it and have the parent sign and save in your files to reference throughout the year.
