When it's time to get your teenager into a ritual of carrying out their chores, sometimes a simple checklist can help organize their progress! I have saved you the time and frustration of creating your own chart. Just edit in your specific chores. The chores listed are the ones I have for my son going into the 9th grade. This behavior rating scale was designed for a student who has already been diagnosed with ADHD or has the same symptoms as ADHD. It isn't a screener to identify possible ADHD. I fit 2 days per page, but if you set your printer to print on both sides of the pages, you can fit 4 on one sheet of paper. I recently started teaching my mentally-challenged cousin about math, time, and money. She is almost 39, so she is not a child. However, her skills are that of a child. I wanted to approach this in a way that didn't seem too juvenile. She understands digital time very well, but not analog. I made up this sheet so I can write a digital time in and she can learn to identify and draw it on the analog clock face. As we continue on, I plan to share the resources I make.
