This is a form I developed for ease of taking notes during the school day while my students are completing activities for their IEP goals. Please see my other specific sheets for academics and behaviors. It is a Word document so you may delete my goals and put yours.

This is a modified number of the day sheet. I use this daily to help my lower ability EC students gain necessary math skills based on the earlier days in the year. It is also enlarged for my low vision students. I also print it on colored paper.
I used this to track student's behavior. Quick and easy report for parents to know how their child has performed behaviorally and academically in my classroom for each week. I send this home every Friday and have it returned the following Monday with Parent signature. Can adapt for higher grades.
Being an Exceptional Children's Teacher, I send home IEP progress reports every nine weeks. I write student's names and attach label to a folded piece of paper and put in teacher's boxes. Please feel free to use and adapt as you wish.

This student information card is GREAT for the BEGINNING OF THE SCHOOL YEAR and includes all necessary information all on one page for quick access. It was created in Microsoft word so it can be easily modified to fit your needs. It includes demographics,NCWISE/ID number, address, phone number, parental contacts, emergency contacts, medical information, allergies, and transportation. I modified it from an adobe reader format I found on classroom timesavers, but it is completely original because

I have an open form of communication in my classroom. I feel like when my students trust me, they can open up to me and in turn will want to be motivated and willing to try new things to be successful. Being in an EC classroom, I am constantly searching the internet for new innovative ways to teach to all learning styles and learning modalities. I want to teach to their needs but also incorporate neccessary curriculum required for North Carolina.

My own education history
Attended Beaufort Community College for two years and transferred to East Carolina University in Greenville, NC. I graduated in 2003 with a BS in Special Education-Learning Disabilities with a concentration in Psychology.
