It was created in Microsoft word so it can be easily modified to fit your needs. It includes demographics,NCWISE/ID number, address, phone number, parental contacts, emergency contacts, medical information, allergies, and transportation. I modified it from an adobe reader format I found on classroom timesavers, but it is completely original because

Teaching style
I have an open form of communication in my classroom. I feel like when my students trust me, they can open up to me and in turn will want to be motivated and willing to try new things to be successful. Being in an EC classroom, I am constantly searching the internet for new innovative ways to teach to all learning styles and learning modalities. I want to teach to their needs but also incorporate neccessary curriculum required for North Carolina.

My own education history
Attended Beaufort Community College for two years and transferred to East Carolina University in Greenville, NC. I graduated in 2003 with a BS in Special Education-Learning Disabilities with a concentration in Psychology.
