Peer Speech Evaluation
This form is for the OTHER students to fill out while the speaker is presenting his speech. I can't tell you how amazing this worked out, even beyond my wildest imagination. You make 2 copies per page. Have the kids cut them, and then as the presenter is speaking, kids "grade" them by circling one of the 3 calvins for each category. Diary pages with picture and writing
I've used this in my 3rd and 5th grade classes for LOTS of stuff. It's very helpful when you have to observe something (ie the growth of a plant) over a few days. We've also used this diary when we started a squirrel and bird feeder outside our classroom. We drew pictures of the new furry friends that came to join us, observed their actions, named them and described their characteristics, etc.

Independent Work Time Self Evaluation
This is a great piece for self reflection. When you give students a project that lasts a few days, this asks them to evaluate how well they used their time during that day. They are welcome to be completely honest (however, you file the ones who say they wasted time all day) and it helps them to prioritize what they need to do the next day, or what is being a distraction for them.

Back to Class Pass
This is a goodie that I use at the start of the year to welcome kids back to school. It's a Back to Class Pass that the kids can use to either a) get 15 points extra credit to Buy Back a Bad Grade (BBBGP) or b) to get out of doing a homework assignment. Kids get really excited to use their pass, and it teaches them responsibility because once the pass is lost, it won't be replaced.

Book Review without story elements
I use this product for my kids to fill out to tell me if a book was simply enjoyable or not. I use it WITH our Accelerated Reader. The review does not ask about story elements (author, plot, characters, etc). It does ask about how they would rate the book to who in class they would recommend it to, who they wouldn't recommend it to, genres that were included, etc. It is a very EASY form to fill out so that I know which books to recommend to others and which books are duds.

Extra Credit Ticket
How do you keep all those points up when kids are turning in extra credit at random times? Use extra credit tickets! I developed this idea about 4 years into my career and it has been a Godsend... When a child turns in extra credit (I usually assign specific assignments/projects up to a specific amount of points due on a specific date), they earn a particular amount of points, and then I write them a ticket. They have to KEEP the ticket SAFE and then turn it in when they want to improve their

Extra Credit Buy Back Bad Grade Pass Reward
This pass is commonly called the BBBGP in my class, which stands for Buy Back Bad Grade Pass. Students have to earn and buy this pass (points, stickers, money), and it's one of the prizes they can earn. You can decide how much the point value of the pass will be worth (some years I have done 15, others it's been worth 25). I laminate my passes, write on them with vis a vis pen and then reuse them once their turned in.

Homework Pass to skip assignment
This is a goodie that kids can earn with points, stickers, etc. and can be used as part of your incentives program. Students gain a homework pass (many of them save up for a few) and can use them on any assignment, as long as they ask me first. I will not accept any pass if a student has not checked with me before using it. They also have to keep the pass and turn it in with the undone (or lost) assignment (teaches responsibility!!)

I love to create. I've taught for a long time, and I am still revamping things to make them more fun, and more interesting to the kids. I am very excited to be apart of this website, as we all know that teachers are HIGHLY underpaid for their time in the industry...
