If you have a student who is struggling in your class, give them this form to fill out. I normally conference with my students and talk about way to improve their grade. I edit this document and have them sign it. If their grade does not improve, I involve the parents and we think of strategies together and all three or four of us sign the form. Use this resources as a resource for when students take a test. When students begin a test, they can dump their thoughts on this sheet before taking the test. During the test, if they need a break, they can use the bottom half of the sheet to reset their brain.

This product helps students study for an upcoming math assessment. I have included a study guide, a study planner, and a pre-assessment template.
