If you have a student who is struggling in your class, give them this form to fill out. I normally conference with my students and talk about way to improve their grade. I edit this document and have them sign it. If their grade does not improve, I involve the parents and we think of strategies together and all three or four of us sign the form. This product has a missing homework form and a missing work form. If students are missing your homework or work, have them fill this out and bring it home to their parents. Some parents do not check their child's grade and this is a nothing way to communicate home. This product also has a form for the end of the quarter and marking period for students to fill out. This form as students write down all of their missing work and asks them when they will make it up.
