Students graph their behavior throughout the week. It is a good visual for them to see which areas they need to improve upon.

This states rules and procedures for pretty much everything done in and outside of the classroom.

Student Information Sheet

The student gets a positive note if the teacher notices the student exhibiting appropriate behavior. The student writes the note to their parent and gets in signed and returned the next day. The note will go into the students file.
