I am a Title 1 teacher so for our Title 1 Reading Night I put together some activities for our students and parents to play. For my K-2 students, we use the BURST program. So, my activities for K-2 pretty much came from that program. My other activities for the upper grades are activities we've done or worked on in class.

This project asks that students create a business. Students must calculate the markup for one product in their business. They will create a discount and calculate the discounted price of one item. Students will also have to calculate the sales tax of the item. Other parts of the project include creating a sample receipt and a flyer advertising their discounted product. Includes cues for what operations to perform.
I created this Fluency Chart for my fifth grade students who were surpassing the 140 wpm that my previous fluency chart had. It's a pretty basic format. It is intended for reading one passage two times. It has space for writing the passage title, words read correctly, and errors.
This data sheet has three sections. There is space at the top for putting benchmark scores (BOY, MOY, or EOY). The second section is for keeping DORF progress monitoring scores (Date, Passage Title, Words Correct, Errors, Retell, Retell Quality) and the third section is for DAZE progress monitoring scores (Date, DAZE Correct, DAZE Incorrect, and DAZE Adjusted Score).
This sheet is meant to be used for keeping track of DIBELS DORF and DAZE progress monitoring scores.
There is a section at the top of the page to put the student's benchmark scores from the BOY or MOY test. Then, there is space for putting the date, passage name, words correct, errors, retell, retell quality, and daze correct and incorrect scores.

To use this graph, fill in the Beginning of the Year (BOY) score for each category and a goal score for that category (it can be individual or the goal set by your school). Then, fill in numbers in the shaded in column on the left to create your graph. The bottom axis is for writing in the date of that progress monitor.
These fast phrases cards can be used for self-study, partner activities, to send home with students to work on with their parents, or can be used in the classroom in a variety of other ways. These fast phrases help build fluency for struggling readers. Files are in a Zip drive. 8 documents with each set in an individual document and one document that contains all 7 sets.

The chart has a space for writing the reading passage. It allows 2 readings of one passage.
I've added space to write the Words Correct (WC) and the Errors for each reading. I find that this helps with keeping data for progress monitoring.
This is similar to my other 160 word fluency chart but this one has a few extras. It has a place to write the date and a space for writing in the number of words read correctly and number of errors that they made in that passage.
It is like my other one in that it is meant for one minute timed readings. There is a place to write in the name of the passage and two columns to fill in for 2 readings of the story. It is in increments of 5.

This score sheet is for a whole class of students. It is meant to be used with the second grade test for mCLASS Math which is a paper pencil test. After you grade students work, simply write in their name and score for each category. This sheet makes putting the scores into the mCLASS website much easier. I also made every 2 lines light gray to help your eyes stay on the right student's name and student's score.
This could easily be changed for other grade levels or for other tests. The mCLASS
This story map allows students to document the Story Title and Author. The map has boxes for Characters, Setting, Problem, Solution, and Theme/Lessons Learned.

This chart counts up to 60 by fives, has a place to write the date and the title of the story being read.
