This is a ready-to-use form for students to add to their high school professional portfolio that they use for their mock interviews. I have my Career and Financial Management (CFM) class ask their teachers, coaches, etc. to fill out this form for their portfolio. Asking for a time-consuming reference letter should be reserved for specific purposes -- college applications, scholarships, etc. Suggestion: at least 3 evaluations should be included in the professional portfolio.
