Bathroom Sign-Out Sheet
I used this in my classroom to keep track of students use of the restroom during my class. It was usually never needed, but occasionally I could refer to it during a parent teacher conference if I feel like the student took advantage of leaving my class as a habit.
I did time in/time out because I had a 4 minute time limit rule.

As a middle school math teacher, I have experienced a variety of settings, classrooms, and schools. Sometimes, due to lack of resources, I have had to rely on creating much of my own materials. Furthermore, I know the importance of being organized yet flexible, engaging yet structured.
Teaching style
I've had the challenge of 33-student classrooms with students with various backgrounds and skill levels. So, being able to quickly assess a big group of kids is important. My shop focuses on providing you bellringers/exit tickets in order for you to be able to assess students' understanding of the material in a quick and informative manner.
These resources should be flexible as to how you use them. Maybe you keep them on one page and collect at the end of the week. Maybe you project them on the board and have students write on their own paper. Maybe you cut them up into slips and collect at the end of each lesson. It's up to you!
I've aligned each resource with Common Core in order to aid you in your lesson planning.
