After completing a book as a class or in small group, students will choose an activity or activities from the "book club choice menu" to demonstrate their knowledge of the material they have just read.

Accurately show how you are supporting your students by documenting the amount of time and type of support you are rendering. This form can be used with inclusion students, behavior students and students who require interventions within subjects.

This is a small desk tag for students to communicate how they are feeling while working. "My Working Light" helps reduce student frustration, classroom interruption and promote student self management.
This is a great way to informally assess who understands the current topic and who needs additional support quickly.
*This is a tool to use for all students (general education and special education) either in whole group or small group.
Sometimes students need time to reflect uninterrupted. This "thinking desk sign" provides children who need to be withdrawn with a "thought process plan" which will allow them to calm down, reflect and rejoin the group when they are ready.
Allow students to have input in how they are going to correct or change their behavior. With this action plan sheet, students can list replacement behaviors for the undesirable ones. For example, a student could write "Instead of ripping my paper...I can raise my hand and ask for help."
By placing the new "wanted" behavior in their own writing they begin to take responsibility and ownership of their actions.
