Note to put in cubbies when a student is missing an item. Write anything on this form: For example: "Please return the following items to school as soon as possible: spare socks and shorts" "Please return the following items to school as soon as possible: emergency dismissal form for office record" "Please return the following items to school as soon as possible: student immunization record" etc.

Sign for outside of prek classroom door. Trace and cut student handprint for parents to notice.
