Fifth Fun

I give this form to my students on the first day of school. It's a great way for me to start getting to know them. It helps me get an idea of their family situation and their attitude about school, and it also gives me a chance to learn about some of their favorite things. These sheets are great to reference throughout the school year. They especially come in handy around a student's birthday if you do anything special for birthdays in your classroom!

This is a handy little sheet I send home at the beginning of every school year. My students are always excited to bring home "homework" for their parents to complete, and these are full of very useful information! This sheet opens up a line of communication between home and school immediately. I can't count how many times I pulled these sheets out of my desk to reference them throughout the school year.
