Thank you for downloading this file. I have used these forms for years and have found them to be essential for organizing information. I send the Student Information Sheet home on the first day of school. You may display the Volunteer Sign-up during your Back-to-School event or on Meet the Teacher day. This file is a great resource for keeping data. It is also an easy way to quiz and grade students on each list. I have also included a sample of the parent letter that goes home at the beginning of the year. Make copies of pages 3 and 4 and attach them to the letter. Then, make copies of pages 5-8 for each student for grading and/or keeping data. I usually make a copy of the scores for lists 1-3, 4-6, 7-9 and 10-12 in order to keep parents informed of their student’s progress.

progress. This also helps me keep a record of skills
and strategies discussed during the year. I hope this
is as helpful to you as it has been to me.
Thank you!
