I made this sheet to make reading teams in my kindergarten class. Before beginning, students need to check over the condition of their book and check if it needs to be repaired or not in order to help teach them responsibility over classroom materials. Students need to complete the form with their partner over the course of several days. The final step is to answer questions about the story from the teacher.
