These simple and short booklets contain several different formats geared more specifically to the 2nd to 3rd grade students in a Catholic School.
Here are the basics of the booklets.

This conference sheet can be used for leading your Paren/Teacher conferences. This form contains a small section for DIBELS Next Benchmark scores. The grade level benchmark goals are listed on each form. The first form is Kindergarten and each a grade level after that. The form is meant for Catholic Schools and does have a Religion section in the grades.
This simple form gives parents a quick look at their child's grades for the midterm. A small message is written into the midterm and a signature slip at the bottom of the page. This is a great form for parents and teachers because it is simple and easy. At a quick glance, parents see their child's grade in every subject. Plus you get two on a page, so less copies to make.

I have a parent/teacher conference form. This form provides a lot of information in an organized manner. I love it because it keeps me organized and allows me to say and touch base on everything that I need to discuss.
