Do you have those students that might need a heads up for the next teacher? At one of the schools I taught of we were required to fill out a transition plan. I took that plan and went a step further. Here are a few questions that every teacher tends to ask about students. This gives a hard copy to the answer the next year's teacher might have. When used correctly it is a great tool for your classroom and school.
