Great for practicing conversation! This is a form for parents to fill out and give to the SLP before sessions. It lets the SLP know what the child did the previous week/weekend, and any plans they may have in the coming week/weekend. This allows for the SLP to practice conversation skills with the child, that is guided by the information provided by the parent!

Additional biographical information
I am a Speech-Language Pathologist who has worked with all ages, but primarily in pediatrics. I have always loved creating, and have made a lot of my own therapy materials over the years, and have decided to finally share them with more speech therapists! No need to reinvent the wheel!
