This form is a print out for teachers to use when using a tattle box. This makes it easier to have slips already made for easy print. I use this with my kindergarten and first grade teachers accompanied with a tattle box.
I made this to post in my classroom and in my slides for my guidance class. It is a good refresher for the students to know their expectations. Also, it is very cute!
