When middle grade, middle school, and high school students see 3-8 teachers in a day. Making sure you gather their input prior to an IEP meeting can be a daunting task and often overlooked. Use this form to gather input from general education teachers prior to an IEP meeting. Simply print this form, record the student's name, IEP date, and your name and place it in the mailboxes of all of the teachers who interact with the given student throughout the day.
There are a lot of versions of this pencil floating around, I made these quickly for my kiddos to each keep in their math folder. They are becoming much more diligent at following each individual step when solving word/story problems! Just print and laminate!
