I created this template to help teachers take meaningful, in the moment anecdotal notes on their students. This template provides a space for observations and a place for instructional decisions based on the observations. This formative assessment is quick, easy and can be done throughout the year to keep track of student progress. These notes can be taken for any subject and are great to reference while doing report cards as well!

I made this graphic organizer to evaluate students' abilities to retell key details from a story. This graphic organizer works with many different books and passages. There are two different versions of the key details graphic organizer and a rubric that can be used to evaluate student work.

I created this template to use while planning weekly centers, to make sure that each center aligns with standards and can be differentiated to meet student needs. This template has space to list the station name, the activity that will be featured, the standards and focus skills, what materials are needed and ideas for differentiation.
