Tired of traditional reading logs? Here is a different style to consider that incorporates choice and a variety of reading and writing tasks. This editable resource has monthly-themed challenges for students to complete. You decide the requirements, the incentives, and how many boxes they may complete. If you have serious readers who just wanna read, I make that count too! This can be differentiated for different groupings or individuals. Includes September through May.
Here's a differentiated way to get students focused on the Common Core reading standards, with choice built in! Bingo Board 1 is designed for more struggling learners, as it has less complex vocabulary and sentence starters. I've used this many, many times for students' independent reading and/or responses as a result of a literature group. Editable to suit your needs.

This is a checklist I use with reluctant writers and/or students who struggle with planning their writing. It helps them to consider the writing space provided (size, style, boxes, lines) before they begin writing. This is especially helpful for students with occupational therapy or handwriting goals or with handwriting needs. It can be printed as-is or shrunk to whatever size needed.

This bookmark gives them lots of options: number line, drawing groups, skip counting, bar diagram, array, etc. It can also be used as a periodic formative assessment! This editable reading log helps teachers and families ensure students are reading at home! This log is adapted to allow different types of reading (self, others, etc). in our classroom, the students each have an individual goal/expectation for how much to read (by chapter, page range, or minutes), so it is not indicated on this log but could be added.
