Where would we be without SOAP notes? As an SLP-graduate student, finding a system to comprehend and balance everything together is tough. Each university has its own way of teaching students to write and interpret SOAP notes let alone the instructors who also have their own system for SOAP notes. Trying to balance between the two all while trying to comprehend what I am doing has led me to this.
