This is a great form for parent teacher conferences. On this form you will be able to add a student's grades, behavior, and add comments. It is editable on Microsoft Word to add or take out anything on the form. It is a great tool for documentation too. At the end of the conference I make a copy of it to provide to the parents. You can add your school's logo on the top instead of the clip art.

You can use this weekly newsletter as a great communication tool between teacher and parent. I used it for grades Kindergarten through 5th. I included information on what the students' were learning, reminders for the week, announced student of the week, and tests for the week. I called it "Peek at the Week". I sent it home on Mondays so parents knew about the upcoming week.

This is an excellent, simple, and to the point form to use to request a conference. It is a letter sized word document. 2 forms on one page. Information includes day and time of requested conference, response section, and available time and date of teacher if parent can not attend the date and time you requested. I copy this on colored copy paper to stand out against other papers in the student's folder.

Designed for avery labels 8160 size 1" x 2 5/8". Can be used for any scheduled conference. I put the label on my student's folder or agenda two days before the conference to remind my parents about our teacher/parent conference. I also stick the label on the students shirt the day before the conference as another way to remind them.
