This is the science lesson plan template I used in my classroom when I taught 5th grade. I really like this format because it is simple and to the point. You can add hyperlinks to experiment videos, include anchor charts, Ticket Out the Door activities. Principals love it because the differentiation piece is on the very front and easy to access.
