A Report Card for the Teacher - Feedback from students

Created by

Students provide feedback to the teacher in the form of a report card. This could be done each quarter, each semester, or at the end of the year. Copy/print as a double sided document and fold in half to make this look like an "official" report card. :) If you like this product, you may also be interested in the following: Teacher Survival Kit Tags Guide to End of Year Letter to the Teacher End of the Year Silly Student Awards Remember to leave feedback when you download this product to

The first page is the version I use in my own classroom. The second page is a more general version of the same form. Please leave feedback when you download this product. Looking for a better way to monitor, track and address student behavior in the classroom? These posters can be used as a visual reminder for students. Students move clips when instructed to do so. In addition, I have included a one page teacher behavior tracking chart and a note that can be sent home with students explaining the behavior system. Enjoy!

Additional biographical information

When I'm not teaching, I'm creating... I love sewing, scrapbooking, photography, home decorating, and designing new math and science lessons. Yes, that's right. Much of my "free time" is spent "working". But it's hard to think of it as work when it's what I LOVE so much!
