A Report Card for the Teacher - Feedback from students
Students provide feedback to
the teacher in the form of a report card.
This could be done each quarter, each semester, or at the end of the year.
Copy/print as a double sided document and fold in half to make this look like an "official" report card. :)

I love sewing, scrapbooking, photography, home decorating, and designing new math and science lessons. Yes, that's right. Much of my "free time" is spent "working". But it's hard to think of it as work when it's what I LOVE so much!
