I created this graphic organizer for my writing stations in my classroom. We use a picture of the day for the week. After they have completed the see, think, wonder page, they will then write a paragraph about the picture and their thoughts. Really gets them thinking! I used a free font on this document that you may not have on your computer.
This is a rubric that I made for our Social Studies Fair. There are two whole pages that equal a 25 point rubric. It is a little bit easier to read with the pictures for each category. The points weighted are smaller so it is easier to add up for the students as well. Enjoy!

This is a form I created for parents to fill out at Open House on their child. The preview shows the lines off center, but when you open it up, everything aligns right. There are certain items that I would like to know specifically that I have included such as: which parent they live with, allergies, transportation to and from school, siblings, etc. I can keep this right by the phone for a substitute as well. This is a chart that I created to organize stations in my room. I departmentalize so I only have the students for about an hour an a half. The first 30 minutes is for the bell ringer and short direction instruction for mini lessons. The other hour is dividing into 20 minute sections for small groups. I work with groups in the second two rotations. There is a section that you can type or write in the student names for each group as well. This document is editable so that you can customize it
