This is a super simple tally form! It's a great resource for observing a student to assess their behavior - recommended for 30 to 60 minute observations. The form is set up using my own "seesaw method" so that classroom teachers, counselors, behavior specialists or administrators can easily tally the side of the descriptor that the student leans more heavily toward. It's also really great for passing along info to a sub or other staff who may be unfamiliar with the student's special interests. Important: This is not a document for storing confidential or personally-sensitive information. It is an informal transfer of key details that broader staff can utilize.
