This is a simple form that I send home with all of my students to get important information about them. I send these notes home, the parents fill them out, and I keep them in a binder in my classroom anytime I need to reference it. Sometimes it takes us a bit of work to stay organized in our classrooms. I have found that having areas in the classroom to store papers works really well. I created these labels for my classroom.
