This form is for teachers who offer opportunities to correct a test for a 70. Print these slips out on bright paper and staple to any test you want you the student to study for and retest. I require a parent signature in order to retest and change the grade in my grade book because I want them to know when their child has failed a test. This also makes great documentation at meetings.

This is a letter you can send home to parents to let them know about an ice cream sundae party reward. I use this reward for completion of the first unit in the Six Traits writing program, which involves the use of an ice cream sundae graphic organizer. There is a place for parents to sign up to send treats for the reward. Also included is a reminder note for those parents who signed up.

I developed these for my 4th-grade team to help students with organizational and study skills. Print them out, laminate them, and have students post them on the fridge at home. I've included a detailed parent letter explaining the purpose.
