This is a daily school counseling/counseling notes log that helps a counselor/social worker keep vague, short notes daily to help with tracking of students. This can be used to later add student notes into a student information system or data tracking system. It includes a space for a name, the time, and notes as well as a general notes box at the bottom.

This lined notes page can easily be printed in large quantities and put into a binder or notebook to keep notes on a student during counseling sessions.
