These are labels to use with Avery 5263 2 in. x 4 in. labels. Perfect for name tags, supplies, etc. There are 10 on each page but I could not figure out how to show that in a thumbnail.

This survey allows the students to give you as much or as little information about themselves that they feel comfortable with. In Part 1 of the survey, they will use different sentence starters to complete sentences in any way that makes sense. Part 2 is the typical "What's your favorite...?" Part 3 is short answer questions where you can dig a little deeper into their personalities, heroes, and lifestyle. They love that is it not just the basic, "What is your favorite..." survey.
