This survey allows the students to give you as much or as little information about themselves that they feel comfortable with. In Part 1 of the survey, they will use different sentence starters to complete sentences in any way that makes sense. Part 2 is the typical "What's your favorite...?" Part 3 is short answer questions where you can dig a little deeper into their personalities, heroes, and lifestyle. They love that is it not just the basic, "What is your favorite..." survey.

This is a survey that can be used to get to know your students at the beginning of the year. I give them to all of my classes on the first day of school. After I collect and read them, I hang on to them until the end of the year. I give them back and the kids enjoy seeing how their answers have changed.
