Student Self Assessment Daily Behavior Checklist
Students can quickly reflect on their behavior in various categories at the end of each day. I send home work for the week, attached to the checklist, for parent signature. I have used this successfully in my classroom for many years. It can be easily tweaked to fit your own needs or areas of focus.
