Therapy Notes for Parents - Session Summary

Cut the form in half to send home with the student (or leave at the day care / preschool). Or log your notes on the computer then email or print for parents. The form is in DOCX format so you can edit as needed. Fill out the "Today, we worked on..." section with brief descriptions of the student's goals (e.g., making the /s/ sound)

I provide many supports in the classroom and in therapy (visual, auditory, manipulatives), as I recognize that students learn in different ways and that multi-modal presentation of information produces the best learning outcomes.
