This is a short exit slip with a graph to interpret and four questions that can be used for a quick assessment.

Students are given a task to design a theme park. They are asked to make a blueprint highlighting the perimeter of multiple objects- both regular and irregular shaped. This comes with a recording sheet, checklist for self assessment, and grid paper-- everything your students will need to complete this task.
