In order to reach my most at risk kiddos and those who are exceeding expectations, I use my MAP data to form workshop (RIT) groups. This is the lesson plan format that I use to keep track of my groups as well as individual progress within those groups. This is not a formal lesson plan and is meant to be used as a quick guide/reference to who belongs where during math and reading RIT times.
I have created 4 different quick and easy formative assessments to be used as exit slips for reading and math. The pack includes 1 math exit slip, 1 reading exit slip, 1 reading response exit slip, and 1 "show your thinking" math exit slip.
All exit slips have been designed for you to easily write in the "I can" statement you are currently working on without the hassle of having to dig through your resources for the standard you are teaching.
This is an informal lesson plan template that I use as a weekly guide. The product is editable for you to customize your needs. Please let me know if you have any questions on how to change things around!
This packet is intended to be used as a reinforcement of multiplying by 2. Cut out and laminate the cards for use to target individual needs and/or during math centers. The worksheet can be used as a quick formative assessment/exit slip.
